Lower limb robotic rehabilitation uses specialised equipment to support repeated walking practice. It may help patients who have significant leg weakness or difficulty maintaining a safe walking pattern after a neurological or orthopaedic condition.
The equipment provides support while the patient practises stepping and weight transfer. It can allow repeated walking practice during a stage when independent walking is still difficult. Robotic therapy is used as part of a wider rehabilitation program and does not replace therapist-guided exercise.
Why and when it is recommended
Robotic gait training may be discussed after stroke, spinal cord injury, traumatic brain injury, and selected conditions affecting leg strength or mobility. It may be useful when a patient is medically stable but cannot walk safely or for a long period without assistance.
Before treatment begins, the team reviews joint movement, muscle tone, strength, balance, cardiovascular tolerance, skin condition, and the ability to stand safely. These factors help determine the level of support and the type of walking practice required.
Treatment
The patient is positioned securely in the equipment, usually with support for body weight. The system guides the legs through a walking pattern while the therapist adjusts speed, assistance, and weight-bearing.
As strength and control improve, the level of support may be reduced. The patient is encouraged to take a greater role in each step. Treatment may be combined with strengthening, balance work, standing practice, walking on the ground, and exercises for the arms and trunk.
The therapist observes posture, foot placement, step timing, weight transfer, and fatigue throughout each session. The program is changed when the patient’s movement or endurance improves.
Recovery and follow-up
Progress may be assessed through walking speed, step symmetry, endurance, distance, and the amount of help required. Patients may gradually move from robotic practice to supported walking and later to walking with an aid or without assistance.
Regular reviews help determine when support should be reduced or other exercises added. The main goal is practical mobility, including safer transfers, improved walking confidence, and greater participation in daily activities.
