VitalStim Therapy for Swallowing Stimulation

VitalStim therapy is a specialised option for selected patients with swallowing difficulty. It uses mild electrical stimulation through surface electrodes while the patient performs swallowing exercises under professional supervision.

Swallowing problems can affect nutrition, hydration, breathing safety, and quality of life. Patients may cough during meals, choke, develop a wet voice, take a long time to eat, lose weight, or experience repeated chest infections. Early attention can help identify safer ways to eat and drink.

Why and when it is recommended

VitalStim therapy may be discussed after stroke, traumatic brain injury, head and neck cancer treatment, or a neurological condition affecting the swallowing muscles.

It may be considered when standard swallowing exercises have not produced sufficient progress or when additional stimulation may support muscle re-education. The decision depends on the patient’s swallowing pattern, medical history, alertness, ability to follow instructions, and general safety.

A swallowing assessment is required before treatment. In some cases, further testing may be recommended to understand whether food or liquid is entering the airway.

Treatment

The speech-language pathologist places electrodes over selected muscles in the neck. The electrical stimulation is adjusted carefully while the patient practises swallowing tasks. Treatment may begin with saliva or dry swallowing and progress to selected food or liquid textures when appropriate. The program may also include posture training, oral-motor exercises, breathing coordination, diet modification, and caregiver education. Some patients may need a videofluoroscopic or endoscopic swallowing assessment. These tests can provide additional information about muscle movement and swallowing safety.

Recovery and follow-up

Progress may include fewer coughing episodes, better muscle control, safer swallowing, and the ability to manage a wider range of food and liquids. Diet changes are made gradually and only after reassessment. Some patients may require temporary alternative feeding support while working toward safe oral intake. Follow-up visits help review swallowing safety, nutrition, hydration, home exercises, and diet progression. The aim is to help patients eat and drink as safely and independently as possible while reducing anxiety for both patients and caregivers.
